About The Book

This book on Hysterosalpingography for AYUSH practitioners systematically covers all the description and explanation needed for understanding and interpretation of HSG X rays.

It starts with a brief introduction to HSG, following normal anatomy of uterine cavity, endocervical canal and fallopian tubes enabling morphological assessment of all the structures in detail. It also includes the indications and contraindications of the procedure, timing of HSG and its significance. The standard and routine protocol to be followed while interpretation is mentioned, so that the significant and minor details are not missed.

There are approximately 85 HSG X ray images given. Initially few examples of normal X-ray images are given for practise followed by abnormal X-rays. The various abnormal conditions included are filling defects - both physiological & pathological, submucous fibroids, polyps, synechiae, congenital anomalies like septate, bicornuate, unicornuate, arcuate uterus etc. tubal occlusions, various changes seen in genital tuberculosis, SIN, hydrosalpinx, T shaped uterus, adenomyosis, intravasation of dye etc.

It is easy to read as well as absorb the contents as it is simplified along with as many images as possible. Moreover, different images of the same condition will enable the reader to get acquainted with the fine nuances of each condition. While coming to the end of this book the reader would feel confident about interpreting HSG images; and that's the purpose of this book.

It is highly recommended for all the practitioners of Indian medicine and homeopathy (AYUSH).

Introduction

Hysterosalpingography or Uterosalpingography is the radiological evaluation of uterus (hystero) and fallopian tubes (salpingo) under fluoroscopic guidance and is mainly used in evaluation of tubal factor infertility. It is primarily a tubal patency test, most commonly used to evaluate fallopian tubes.

Though it is a diagnostic method, it has a therapeutic effect as well. This is due to the flushing of inspissated mucus and debris from the tubal lumen leading to fertilization and conception. This being the reason behind spontaneous conception or enhanced fertility rate following HSG.

Hysterosalpingography (HSG) is preferable over other alternatives which are comparatively more invasive in nature like laparoscopy, hysteroscopy, sonohysterography. With the advent of USG and laparoscopy, it remains only a baseline investigation to rule out tubal block in cases of infertility. HSG helps in detection of intrauterine abnormalities such as submucous fibroids, intrauterine adhesions and in checking the patency of the fallopian tubes (tubal obstruction, hydrosalpinx, pyosalpinx etc.) and a defective internal cervical OS causing abortion or premature labour. Hysterosalpingography is valuable in the diagnosis of tubal disease such as tuberculosis. However, in the presence of active disease, it can lead to dissemination or activation of the disease and prove dangerous.

To conclude, HSG is a minimal invasive procedure, superior to laparoscopy in detecting intrinsic tubal and uterine pathology and complementary to laparoscopy in evaluation of infertility.
